Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 05/28/2019 and 06/04/2019 regarding ratio. The inspection was initiated on 06/13/2019 and completed on 06/17/2019. On 06/13/2019, the inspector was on site from 7:52am to 11:16am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. In addition to the original complaint, there was also a violation found during the inspection regarding children's records. See the violation notice for more details. Description: Based on review, the center did not ensure that one of two children's records contained all of the required information. Evidence: The record of child #1 (start date: 2/11/19) did not have documentation of the child's proof of identity.

Description: Based on observation, the center did not ensure that when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group applied to the entire group. Evidence: The inspector observed up to 14 children, ranging in age from two to four years old, in the green birds classroom with one staff supervising the children from 7:54am to 8:04am. During interviews, it was revealed that the orange birds (twos), yellow birds (threes) and green birds (fours and fives) are regularly combined in the morning from 7:30am to 9:00am. The ratio for the regularly combined group is applicable to the youngest child in the group. The youngest child observed in the group on the day of the inspection was two years old. Therefore, the applicable child-to-staff ratio was 8:1.
